This guide will show users how to correctly fill out a claim or how to use the eForm
Step 1: Login to the account

Click on the necessary form. Example: click on Once you do so, a new tab will open.
Step 5: If the Approving Officers are set up already then skip this step. If not please choose from the drop list the approving officers. Note: Do not leave it blank, if no Approving Officer please select NONE.
Step 6: After filling in the information of the CLAIM (it will be different per Claim Type).
Step 7: If needed to attach a file please scroll below and click a table will show up. Click to attach the file. Once done click . If all files are attached, click .
Step 8: If you are done with the filing click or if you wish to return to the form at a later time, just click
